Doesn't balancing on destination ip address only work if you are doing transparent proxying?
Because squids don't do transparent proxying. So for example, when my squids get the request it would be of the form:
GET http://www.yahoo.com/ HTTP/1.0
The load balancer would not know the destination IP of the site being hit. Would the DH load balancing algorithm work for non-transparent proxy setups?

Any algorithm balancing the load based on the destination IP.
