Hey Peter,

Regarding the question about LDAP I cannot answer since I am not
familiar with this helper but I am sure others are.
I noticed that you are using squid 3.4.0.3 which is basically a beta
version.
You should consider using a newer version such as stable 3.4.5.
There are many bug-fixes since the version you are using.
