Aha. Possibly. If the URL being changed by that component?
The transaction is being labeled as internal on arrival, but the normal
processing is done right up to the point of locating the icon in cache.
It should always abe a HIT since the static objects are "served" by
directly loading into the cache on startup.
Maybe cache rules preventing the small icon *.png being stored?

In the /var/www/squid or /usr/share/squid/icons (aka
--datadir=\${prefix}/share/squid3 ) I think was the old location. Looks
like something is there so it should be served.
