The "connection refused" indicates either squid does not listen on port 3128
of localhost (127.0.0.1) interface (most likely) or it's blocked by
firewall.
He either should access the squid using other IP on which squid listens on,
(cachemgr.conf can do that) or allow squid on localhost interface.
cachemgr apparently won't be allowed from other than localhost address, so
in the first case he'll need to allow manager access from other source
address.
