Extreme indeed. But usually seen (by me at least) when squid logs exceed
the size of the drive. In which case cache.log will not contain the
crash message (that failed to write). If the drive is 100% full delete
something and restart squid.
If the drive is under 100%, check cache.log for the dying message about
what went wrong.
