Well don't you think that your first level directories value of 144 is quite high. Why don't you use something like 64 or 72? But I really don't think that this is causing your high median service response time.

Which squid version are you using and what is your operating system?
- From my experience, I don't think that your service time of 421 milliseconds is really a problem.
It really depends on how fast your internet link is and what your users are browsing?
When you got the service time of 421 milliseconds, how many users were actually using the proxy server? You also have to check what content or sites they were browsing?
Do you get this service time warning during your non-peak hours?
Now, if someone was using your proxy server to surf a particularly slow site, then this explains the high access time limited to one or two particular sites only.
During peak hours, these slow site access time gets average out with
those "faster" sites, thus giving it a lower and more accurate average
response time.
In fact, this was once asked on this mailing list before but your case could be different.

I am just assuming things because I really don't have any idea of your proxy server setup and how fast your internet link is. What you consider high is considered low for someone using a satellite link like me.
