Are you sure it's Squid who is dead and not your networking? You logs
indicate Squid is running fine, but it's connections to the requested
servers gets nowere.

Anything in your system messages?

When Squid is "dead", does it work browsing to the outside using a
browser on the Squid server but without using the proxy?
