[squid-users] Squid does not log user access

I've configured authenticate-program tag to allow/denied users to access
the internet.
The problem that i'm having is that, in some cases, squid is not logging
the user's name in the access.log file. This problem doesn't happen
always. I mean that some user's name are logged but others (that should be
logged) aren't.
Can someone help me?
