How did you install squid ?
Have you verified that ncsa_auth is at /usr/lib/squid ? Depending on the
installation method you chose, it might well be somewhere else. How does
your /etc/squid/proxy_users look ?

If you seek help in a public forum, it would be wise to leave out such
legalese. Obviously you posted this mail in a public list, so this is
meaningless anyway and lengthens the message unnecessarily for all list
users. Or what does "distribution prohibited" mean in the context of an
open mailing list that will be archived searchable in several public
archives ???
