First upgrade to a current version (2.5.STABLE9 is the current release).
The version you are using is known to exibit this problem if certain
patches had been applied (early versions of the concurrent_dns_lookups
patch): http://www.squid-cache.org/bugs/show_bug.cgi?id=852 but it is not
clear from your report if any patches had been applied.
second if the problem persists, file a bug report per the instructions in
