Yes.. and matches what you see.

If you look at the headers of the cache hit you gate before it indicated
an age of 228519 seconds (or "merely" 3808 minutes) which is within most
of your refresh_pattern settings above.

In addition by using a min age greater than 0 you make Squid cache very
aggressively. See the comments on the refresh_pattern directive usage.
