I think that's because the Cache Manager tracks the root process,
which as you observed isn't restarting (keeps same PID). I have
noticed that a squid -k reconfigure (which triggers a restart in
cache.log) does not affect this.

Nothing - all it means is that Squid's cache is over the configured
limit. Squid will correct that on its own by dumping older objects
from the cache.
