A helper is a separate program helping Squid with some kind of job. In
case of authentication the job of the helpers is to verify a given
username+password combination towards a given backend database (Local
file / LDAP / NT DOmain / PAM / etc). Each kind of backend database
requires it's own helpers, and in some cases there is different flavors
of helpers for the same backend database.

Yes, as most the helpers are shipped with the Squid sources so it is
easiest to compile the helpers while you compile Squid.
