YOU CAN NOT USE AUTHENTICATION IN A TRANSPARENT PROXY. IF YOU WANT TO
USE AUTHENTICATION YOU MUST CONFIGURE THE BROWSER TO USE THE PROXY.
If the above is not sufficient to get you on track, see squid.conf,
Squid FAQ and cache.log while trying to use such configuration as they
all contains documentation on this.
