The script its a loop, it never finish ... it send an ERR or OK mess and
wait for another answer ...
It waits for: username pass (user pass) like the squid_auth_ldap ... mmm yes
it can auth one at time ...
I'm write my own script because I'have some problems with the built-in
helper ...
(The script sends ERR\n or OK\n, its ok?)

When you tested from the bash shell, did the script exit after
printing OK
or ERR, or did it wait for more input? The authenticator needs to loop
so
that it can accept any number of username/password pairs, one at a
time.
Also, why did you write your own? Why not use the built-in helper?
