/usr/local is an odd place to have the smb_auth binary.
Run "which smb_auth" from the command line - is it in
/usr/local, or somewhere else?
If it's somewhere else, change your squid.conf and
try it again.
Also, what are the permissions on smb_auth? This:

makes me wonder if Squid is unable to run the program.
Make sure the program is executable by the user Squid
runs as.
