The pam_ldap module got confused when your LDAP server failed. If should
recover automatically, but maybe not.. (if not, file a bug with the
relevant pam_ldap module maintainer. this is not a Squid problem)
Btw, why are you using PAM for LDAP authentication? I would recommend
using the LDAP helper for Squid if your accounts are stored in LDAP.
