I have quite a few ICP queries logged, with UDP hits and misses.

I have tried looking for a patern to the TIMEOUT_DIRECT's but have been
unable to find any. All my clients pages are being retrieved, so I don't
know if it is just a cosmetic issue.

Do you have any ICP queries logged? If not there is probably something
blocking the ICP packets from reaching the other Squid..

And does your icp_access allow for ICP queries?
