i use ntlm_auth too.

and sometimes authentication window pops up.

authenticate_ttl 2 minutes

refresh_pattern ^gopher: 1440 0% 1440
refresh_pattern . 0 0% 0

connect_timeout 240 seconds
client_lifetime 10800 seconds

Which ntlm helper are you using?

SMB (ntlm_auth) or winbind (wbntml_auth)?
