All of the auth_param directives are verified, and wb_ntlmauth does show the
number of children in the conf as processes. I am not really sure about
cachemgr as I am not familiar with it. It is the .cgi for webmin that you
are referring to correct?
Is there another way to check?

IE should be returning a dialog that includes username/password AND domain.
Sounds like IE isn't aware the squid does ntlm.
Sorry if these q's seem too basic, but they need to be ruled out:
I assume there are auth_param directives in squid.conf for both ntlm and
basic helpers?
Does wb_ntlmauth show up in ps?
Are there any NTLM stats listed in cachemgr?
