no-cache set by the server cannot be overridden at the moment. A bit
too sensitive from a RFC perspective.. plus dealt with at a slightly
different location in the code than the refresh checks..
Adding a refresh_pattern override to these kinds of cache busting
techiques needs a bit of coding..
