Hi,
could this be a dns-problem? After the TT from the ntlm-helper the
browser-client sends dns-requests to the dns-server, an the last packet is
an ICMP-Destination unrechable from the client to the dns server.

This is only half of an NTLM handshake - the TT is the helper providing a
challenge to squid, there should be a KK after that, if there is not, then
the client is not trying to complete the login for some reason.
