Hit Ratios: Total hit ratios for both disk and memory

Memory Hit Ratios: Percentage of the Hit Ratio that was of type TCP_MEM_HIT
Disk Hit Ratios: Percentage of the Hit Ratio that was of type TCP_HIT

Now this is obviously incomplete and is why Memory+Disk < 100% in the
statistics. This should be fixed if someone cares. Patches are most
welcome. The culpit is clientUpdateCounters() in client_side.c. See the
Squid FAQ for a complete list of all log tags and their meanings. The
function isTcpHit() is also a good guidance to the one fixing this up..

Byte & Request is what they say. One type is counted in bytes, the other
in requests.
