I/O errors?
page faults is not errors, it is a function of modern OS:es way of managing
memory.
You seem to have the same problem, serious shortage of memory. How much
memory do your system have?
According the the cachemgr output your Squid alone is using 150MB of memory,
which seems pretty alright for the amount of disk cache you have. Add to this
the memory requirements for any other components.. I would use at least 192MB
of memory for this size Squid, probably 256.
