Did you also notify when reading the documentation that pam_unix requires
arguments when using shadow passwords?
For a working configuration, look at any of the existing PAM services and use
them as a template for your "squid" service. For example, it shold work just
fine to simply copy the "login" service definition to "squid". There is
little point in giving a "working" example above what is already provided, as
what composes a "working" example is different depending on the actual setup
of your OS, making your OS setup a much better "example".
