i had this problem once.. when i was upgrading from Squid 2.3 STABLE3 to Squid 2.3 STABLE5 .. it looked for the 00 directory, but failed to find it... i simply rm -rf'ed my /var/squid/cache and, a restart of squid caused it to setup the directories it needed, including the elusive 00 ...
