this should not happen :)

i tried test case #1 at our site. works like expected. all files were
rejected.

we're using squid 2.4s2, interscan 3.01 all on redhat 6.2.

are you sure that squid sends *ALL* requests to interscan ? check this
in the interscan log. there you should see requests for 'eicar_com.zip'.
and did you configure interscan to scan all files?
