I have tried to use several authentication methods but can not get anything
to work. Is there some more detailed documentation on how to set up the
authentication_program component. I have followed the instructions on Squid
FAQ but the authenticator will not accept the password.

Are you using ncsa based authentication or something else?
Because Im using ncsa_auth my squid log logs username and not ip.
I havent specified for it to specifically do that anywhere in squid.conf

Subject: [squid-users] IP address v Username
How do I get Squid to log the username rather than the ip address. All
entries in the log file are against the ip address of the client and I would
like to be able to view reports with stats against the username.
I am running, Squid Cache: Version 2.3.STABLE4
