Like the attached? (Done against CVS HEAD).

Henrik/Duane - if this is acceptable, could this go in? It's a trivial
patch, so there shouldn't be a need for a separate branch.

Details:
Alter reply_body_max_size to accept http_access style allow acl lists,
allowing per request customisation of the body limit.
