Mostly this is because the lack of permission set for the Squid pid lock file. Find your pid_filename file in Squid.conf and make sure Squid process's owner have write access to it. If this does not solve your problem, what your cache.log says???
Or try to start squid with -NsYd1 option and see the status in terminal window.

I'm wondering why my squid dones't work anymore...
it was working... i've made no changes to my squid-box (running linux slackware current, kernel 2.4.4)
after... no reason no one can use the proxy... no firewalling rules.... access.log logs nothing more...
and... if i try to restart squid i get:

After... if i KILL -9 squid and restart it... it works... WHY ?
