You either did not specify the correct "Cache host/port" parameters, or
Squid is not running.

"Cache Host" is the host where Squid is running (IP where it is
listening for requests) (i.e. the IP of the Squid server, or what you
have specified as listening address before the port number in http_port
in squid.conf)

"Cache Port" is the port on that host where Squid listens for http
reqeuests. (i.e. http_port number from squid.conf)
