For NCSA:
1- src/auth_modules/NCSA/make && make install

Squid.conf:
uncomment this line: authenticate_program /usr/local/squid/bin/ncsa_auth
/usr/local/squid/etc/passwd and read FAQ for further information how
implementing authneticaiton you can.

For htpasswd:
If you have installed apache on your box. You have already this program. So,
u can creat passwd file in /usr/local/squid/etc by using htpasswd.

#htpasswd -bc passwd user1 pass1 user2 pass2 ....

For chpasswd installation:
First read README file please.
#./configure --enable-cgidir= where your cgi-bin is. In my box
/home/httpd/cgi-bin/
#make
#make install
Then edit your chpasswd.conf.
