I'm clear on the symptoms. I'd like to help... but we (anyone wanting to help) need some details on the errors.
1) Where does this error appear? - In your browser? on the console of the squid server? in your syslog? Somewhere else?
2) what appears in your access.log and cache.log?
3) have you turned up your debugging and then checked the cache.log after doing a test case?
