./client -t 1 http://www.hotmail.com
./client -t 2 http://www.hotmail.com
is one way. You can also look at the returned headers which should
include each proxy on the way. Note that SSL will go direct from squid
to the origin server.

In access.log - what error code does it show? i.e a 400/ a 500? What URL
is the error occuring on (from the squid side, not what the client sees
in their address bar.

happens every time) the ip that squid logs in access.log for the failed
request? Is the firewall denying or timing out or dropping any NAT
