[SQU] fake_auth question

In squid.conf I have:
authenticate_program_ntlm /usr/local/squid/bin/fakeauth_auth

for ntlm_auth I needed domain\bdc; I'm assuming I don't need any parameter
for fake_auth? Is that right?

Also, in access.log, I still get a few denied entries before I get
"authenticated". I thought that those "denied" entries would not appear
because there is no real authentication.
