I don't remember the exact version of squid which started doing this,
but in the recent versions of squid:

974643025.280 957 203.56.168.69 TCP_MISS/200 4838 GET http://www.squid-cache.
org/ - DIRECT/204.144.128.89 text/html

.. the actual host IP accessed is logged with the DIRECT/ part.
