cache_peer_domain isn't required if you use always_direct, as
always_direct forbids Squid from using any kind of cache peer while
forwarding the request.
cache_peer_[domain|access] is only required if you want to select
different sets of cache peers depending on the request. For example if
you have one parent which should be used for .com only.
