This was a new one.. The above is a contradiction. The FD does not have
any handlers installed (all three are NULL). It beats me how it could at
all be considered..

No. Sorry. It simply should not happen.
Have you changed anything else, like tuning the number of
filedescriptors available?
Did you run "make clean" after applying the patch or tuning the OS?
To make sure, please run "make clean; make install", and restart Squid.
