If you are using IE <= 5.0 yes. I am told however that the
latest/greates version of IE (5.01?) does assumes there is always a
cache.

Netscape browsers does not make the distinction and always assumes there
may be a cache in the path somewhere.

Yes, doing so tells the browser that there is a proxy/cache in the path.

If you have this problem (reload not working, shift-reload does) with
Netscape browsers as well then there most likely is a date related
problem. A plain reload verifies if the document has changed and no
update will be seen if this verification indicates that it is not
changed, a shift-reload always downloads a new copy of the object.

To tell you in detail what is happening, then enable log_mime_hdrs in
squid.conf, download a page, modify the page and press reload followed
by shift-reload (allowing the browser to update the display in between).
Then send me the resulting three (long) lines for that page from your
access.log.
