New delay pools patch

There's another delay pools patch (hopefully the final one, assuming
the next Squid-2.3.DEVEL release comes out with a fully fixed delay
pools).

From a Squid-2.2.STABLE5 with none of my patches applied (I recommend you
apply at least the bug fixes on http://www.squid-cache.org/ as well as my
patch), here is the patch to apply:

This mostly affects people using poll() who could have seen crashes in some
cases - ie, users of Linux 2.2 or Solaris and delay pools.

Thanks to jani@helou.net for reporting this problem and providing a stack
trace to help debug it.
