Same problem over here on 2.2STABLE4
(b.t.w. no part of a hierarchy).
I set visible_hostname to my FQDN, squid -k reconfigure.

Icons are located in etc/icons, group and user owned by squid, file
exists, enough permissions (default by the make install).
Tried with and without the setting icon_directory.
Anyone who can help me out ?
