Problem authentication with squid 2.2.stable2

Hi all,
I,m currently running squid2.0.patch2 with no problem on AIX 4.2.1.
I,m install squid2.2.stable2 but authentication not work correctly.
When i access to squid i receive the authentication dialog. My
user/password is correct and squid identify this, but squid send me page
error "Access Denied".
In access.log the record with error is:
924855978.453 23 100.2.100.10 TCP_DENIED/403 984 GET http://www.csi.it/
ganio NONE/- -
If i mistake a password the record is:
924861636.958 20 100.2.100.10 TCP_DENIED/407 1414 GET
http://www.csi.it/ ganio NONE/- -
My squid configuration regard authentication is:
authenticate_program /home/squid/bin/ncsa_auth /home/squid/etc/passwd
authenticate_ttl 300
acl user_passwords proxy_auth /home/squid/etc/passwd
http_access allow user_passwords
http_access deny all
icp_access deny all
miss_access allow all
proxy_auth_realm Accesso a Internet tramite proxy
