The main problems are (of course) stupid non-cache-aware chatservers that
use some techniques that don't trigger squid not to go through parents if
the user logs in and "chats". Because of that it may happen that different
requests from the same client (or nick) come from different caches.
Tell your squid using clients to add "always_direct" ACLs for such chat
servers. At least that works for the server's we have (had) problems with...
