This is perhaps a known problem. Squid recovers automatically when it
happpens. The known problem is that there is a race condition in store
maintaince that can cause filenames to be reused while they are being
unlinked.
Unless you are peering with other caches and it happens on a request
from a peer it should not have any noticeable sideeffects aside from the
error message in cache.log.
