TCP_REFRESH_HIT is when Squid validated the cached object with the

1. Object has expired
2. refresh_pattern says it has to
3. The user pushes reload in their browser.

TCP_REFRESH_HIT is when the origin server was contacted to verify that
the cache is up to date. Pure cache hits are marked as TCP_HIT.
