This message is seen when never_direct is active, and Squid failed to
find a useable parent. Perhaps you are bitten by the ICP bug where Squid
declares peers as dead if there is not enought traffic. Try adding
"no-query default" to the cache_peer line of your parent.

If you don't have any parents, then remove any references to
never_direct.
